BigBoss ฟื้นขึ้นมาที่โรงพยาบาลแห่งหนึ่งจากการบาดเจ็บรุนแรงขั้นโคม่า ที่ทำให้เขาหลับไปนานถึง 9 ปี ผลจากเฮลิคอปเตอร์ตกคราวนั้นทำให้เขาเสียมือข้างซ้ายไป หลังจากที่ฟื้นขึ้นมาได้ไม่นาน กองกำลังไม่ทราบฝ่ายก็บุกยึดโรงพยาบาลหวังจะสังหาร BigBoss เขาต้องพยายามฝืนร่างกายที่ยังไม่ฟื้นตัวดี เพื่อหลบหนีออกจากโรงพยาบาลอย่างทุลักทุเล เพราะศัตรูได้ส่งกองกำลังมาถล่มโรงพยาบาลชนิดจัดเต็ม แต่ BigBoss ก็หนีรอดออกมาได้โดยการช่วยเหลือของ โอชาลอท (Ocelot) อดีตสายลับที่เคยทำภาระกิจร่วมกันสมัยที่เขายังเป็นเน็คเค็ทสเนค ซึ่งครั้งนี้โอชาลอทได้ถูกใครบางคนจ้างวานให้มาช่วยพา Big Boss หลบหนีและฟื้นฟูกองกำลังขึ้นใหม่อีกครั้งภายใต้ชื่อ Diamond Dog เพื่อตามล่าคนที่มาทำร้ายเขาและพวกฟ้อง หลังจากติดตั้งมือซ้ายจักรกลเสร็จ ภาระกิจแรกที่ต้องรีบทำคือการชิงตัว รองผบ.คาซ กลับมาเพื่อฟื้นฟูกองกำลังให้กลับมาแข็งแกร่งอีกครั้ง
